申込書の記入が完了しましたら、LINEにて担当カウンセラーへ、お名前と生年月日を添えて、申込書の記入が完了したことをお知らせください。

Once you have completed the application form, please contact your counselor via LINE and let them know that you have submitted the form, including your full name and date of birth.
